The Setup
Washington moved on prices this week. HHS finalized a rule forcing insurers to publish usable negotiated rates, the FTC warned about two dozen companies over bad transparency data, and PhRMA sued to block the GLOBE Medicare pricing pilot. In markets, Vaxcyte's Phase 3 win hit Merck and Pfizer, Humana rallied on an appeals hearing, and XBI fell 4.7% as the 10-year held above 5.3%. Star Ratings, UnitedHealth earnings and CPI all land next week.

Next week matters more than this one: Star Ratings, UnitedHealth earnings and CPI hit within days of each other while the 10-year sits above 5.3%. Hold payer exposure until those prints are in, and treat the GLOBE lawsuit and transparency rule as long-run margin pressure, not near-term earnings risk.
01The transparency rule is a slow-burn problem for insurers, not an immediate earnings hit. Roughly $400 million in one-time compliance cost is small spread across the industry, but CEO-level attestation and dollar-amount rates make the data usable by employers, brokers and the FTC, which is already sending warning letters. Over 12 to 24 months that shifts negotiating leverage toward self-insured employers and away from carriers and high-priced health systems.
02The drug-pricing fight shows how little the GLOBE model now matters. HHS cut its own savings estimate from $11.9 billion to about $440 million over seven years after exempting companies with White House deals, so PhRMA is suing over precedent rather than near-term revenue. Investors should read the lawsuit as a legal hedge against future most-favored-nation expansion, not a threat to 2027 earnings at Pfizer, Lilly or Novo.
03Vaxcyte is this week's real market-moving clinical event. A 31-valent vaccine that matches Prevnar 20 on every shared serotype threatens Pfizer's largest vaccine franchise, but the 2028 filing date, two misses against Merck's Capvaxive and a likely equity raise explain why the stock gave back a third of its gain in one day.
